Spay/Neuter Form: – Community Pet Center

Dogs weighing 50 pounds or more will not be eligible for our spay neuter transport. *Effective April 1. If you believe that your dog weighs 50 pounds or more, you must bring your pet to the Community Pet Center’s office to be weighed prior to pre-scheduling an appointment with our Low-Cost Spay and Neuter Program.

Additional Information: Please bring any vaccination records you have for your pet/s on the day of the surgery. Make sure your pet/s do not have any food or water after midnight the night before the surgery. ALL cats must be in a plastic carrier; sanitized crates will be provided for dogs.
*If you miss your appointment, you will not receive a refund as we will be charged the full amount of your pet/s’ surgical appointment/s. If you want to re-schedule; you will need to pre-pay.

After this form is submitted, stop by the CPC carport on a Tuesday or Friday between 10 am & 12:30 pm to prepay and secure your pet's appointmvient. SPAY & NEUTER APPOINTMENTS * Dates subject to availability. We accept cash (exact amount) and money orders. Cost: includes 5 services. Due to a price increase at Animal Allies prices as of July 1, 2024 *$100 for cats and $110 for dogs*(Dogs must weigh 50 # or less less to qualify).
